Referral Process

All initial appointments require a referral

Our doctors are retina specialists. To refer a patient to us, you can either use this referral form, or use your own that you typically use.

Your patients will be assessed at the Macula Clinic that is closest to them. These clinics have access to advanced diagnostics. If your patient is diagnosed with a retinal disease that can be treated with intravitreal injections on the day of assessment.

If your patient is diagnosed with a retinal disease that can be treated with intravitreal injections, we can offer those injections at our clinics in Kingston or at one of our part-time clinics in Ajax, Belleville, Brockville, Port Hope, the Perth/Smiths Falls area, or Orangeville.
